Activities Assistant - Oakdale Nursing & Rehabilitation Center (Oakdale) - Per Diem

Job Description

Assists with the implementation of a comprehensive, planned program of activities designed to meet the special needs of residents in our Care Center/

  1. Assists with planning and organization of activity programs:
  • Contributes to plan for individual residents.
  • Welcomes input from the resident or family regarding activities.
  • Assists with planning and leading of group activities and one-on-one activity visits.
  • Assists with resident transportation to and from activities.
 
  1. Alert to changes in residents physical and mental condition:
  • Reports to the Activities Director any concerns regarding residents who may be potentially at psychological/psychosocial risk.
  • Contributes to information regarding resident activity involvement to Activities Director.
  • Contributes verbally to activity progress notes.
 
  1. Assists with evaluation of resident activity program:
  • Documents resident response to activity programs in daily charting.
  • Reports to Activities Director any feedback from residents, families, nursing staff and other members of the health care team regarding activities.
 
4.   Successfully demonstrates competency of skills required for department specific compliance.
 

 
        
           
 

Job Requirements

Minimum Education:
Completion of a rehabilitation/recreational therapy course work
High school graduate or equivalent required
 
Minimum Experience/Skills:
Two (2) years work experience in a related field working with long term care patients
Demonstrated creativity and initiative in activity planning
Able to perform basic mathematical calculations
Able to effectively communicate and follow instructions, written and verbally
 
Licenses/Certificates/Credentials:
Current BLS certification
 
Physical Demands:
Prolonged, extensive or considerable standing/walking
Lifts, positions, pushes up to 50 pounds
Considerable reaching, stooping, bending, kneeling, crouching
Manual dexterity and mobility
Ability to move quickly
 
Vision Requirements:
Ability to adjust vision sufficiently to perform duties
           
Working Conditions:
Regularly exposed to the risk of contagious and blood borne diseases
Subject to varying and unpredictable situations.
Exposure to unpleasant elements (accidents, injuries, illness)
Emergency and crisis situations
Subject to irregular hours
Increased stress due to multiple calls and inquiries
Occasional exposure to radiation hazards, radioactive substances and biohazard material           
Occasionally requires time away from the office
